LTC SPARK District Check-In
Created for districts to use on an ongoing basis, these forms will help administration monitor the social-emotional and physical wellbeing of their students and families. Two forms are available and both can be edited to fit the needs of the school/district. The first is for quick, weekly/biweekly input from parents and caregivers, focusing on family wellbeing. The second is an in-depth and involved survey that can be administered once a quarter, trimester, or semester. This survey will provide administrators a better look at overarching parent and caregiver concerns.
